Partner Business Agreement

1. SERVICES INCLUDED

For the one-time investment of $499.00 USD, the partner receives a telehealth business package:

• Collaboration with Escape Zero Gravity Massage and Weight Loss to power and operate the partner's telehealth business under a partnership model.

• A professionally designed, fully functional landing page website branded for the partner's business.

• Ongoing technical support from the service provider for platform use and website maintenance.

• Onboard training, and setup guidance to ensure the partner is operational.

• Weekly coordination meetings to support marketing, and other strategies to support sales.

• Access to partner hub to  create and submit orders for medication fulfilment. 

2. INVESTMENT & PAYMENT

Setup Fee: $499.00 USD, due in full upon signing this agreement. Payment constitutes acceptance of all terms herein.

Monthly Recurring Fee: $299.00 USD per month for standard access to telehealth products. This agreement is month-to-month with no long-term commitment required. Monthly billing begins upon website or products being available online and continues until the agreement is terminated in accordance with Section 8.

4. CLIENT RESPONSIBILITIES

•  Provide accurate business information, branding assets, and content needed to build the website.

•  Use the platform only for lawful purposes consistent with telehealth services.

5. PARTNERSHIP & MUTUAL COMMITMENT

This agreement establishes a collaborative partnership between you the partner and Escape Zero Gravity Massage and Weight Loss. Service provider is committed to equipping the partner with the tools, platform, and support needed to operate a telehealth business successfully. Both parties agree to work in good faith, communicate openly, and act in the best interest of the partnership. You the partner retains full ownership of their business brand and client relationships, while service provider contributes the telehealth infrastructure and ongoing operational support that powers the business.

6. LIMITATION OF LIABILITY & DISCLAIMER

Service provider does not guarantee specific business revenue, partner volume, or regulatory compliance outcomes. Service provider is not responsible for anything outside of their immediate control.

7. REFUND POLICY

Due to the digital and service nature of this offering, all sales are final. Refunds will not be issued after payment has been made for the one-time investment.

8. TERM & TERMINATION

This Agreement is effective upon execution and continues indefinitely unless terminated. Either party may terminate with written notice. 

Payment requirement for continued service: Monthly payments of $299.00 must remain current to ensure uninterrupted access to telehealth products and services. In the event a monthly payment is not received by the due date, service provider reserves the right to suspend access until the account is brought current. Failure to cure a missed payment within 3 days of the due date may result in immediate termination of this Agreement. To avoid service interruption, auto-pay enrollment will be established to support monthly payments. Partner can cancel at anytime. Cancellations must be made prior to upcoming recurring payment taken effect. 

9. ENTIRE AGREEMENT

This document constitutes the entire agreement between the parties and supersedes all prior discussions, representations, or agreements. Modifications must be in writing and signed by both parties.
