This Telehealth Services Agreement ("Agreement") is entered into between Iris Telehealth Solutions ("Company") and the undersigned client ("Client").
1. Services
Iris Telehealth Solutions provides telehealth business support services, including but not limited to program setup assistance, administrative support, vendor coordination, operational guidance, and related telehealth consulting services.
2. Monthly Fee
Client agrees to pay $299.00 per month for services provided under this Agreement.
3. Payment Terms
Payment is due monthly in advance. Auto Pay will be required.
Services will remain active only while the Client's account is current and paid in full.
Failure to make payment may result in suspension or termination of services until the account is brought current.
4. Term and Cancellation
This Agreement is month-to-month.
Either party may terminate this Agreement at any time with written notice. No refunds will be provided once the Auto-Pay deduction has been placed.
No refunds will be provided for partial months or unused services.
Client acknowledges and agrees that all payments made to Iris Telehealth Solutions are non-refundable. Due to the immediate allocation of resources, onboarding, administrative work, consulting time, setup services, and business support provided, no refunds, credits, chargebacks, or partial reimbursements will be issued for:
Monthly service fees.
Setup or onboarding activities.
Partial months of service.
Unused services.
Voluntary cancellation by Client.
Suspension or termination resulting from Client's actions or non-payment.
5. Client Responsibilities
Client agrees to:
Provide accurate information necessary for service delivery.
Maintain compliance with all applicable federal, state, and local laws and regulations.
Cooperate with reasonable requests needed to facilitate services.
6. No Medical Services
Iris Telehealth Solutions is not a medical provider and does not practice medicine, prescribe medications, diagnose conditions, or provide medical treatment. Any medical services are provided solely by licensed healthcare professionals operating independently of Iris Telehealth Solutions.
7. Limitation of Liability
To the fullest extent permitted by law, Iris Telehealth Solutions shall not be liable for any indirect, incidental, special, or consequential damages arising from the use of its services.
8. Entire Agreement
This Agreement represents the entire understanding between the parties and supersedes all prior discussions or agreements.
